If you have a questionnaire that has not appeared for a referral, the probable cause of this is the questionnaire has been set to appear for a specific number of days after the referral was set to ‘participating’ state; For example; A 3 month evaluation questionnaire has been set up to appear 90 days after the referral has been set to participating.

To adjust the days in which the questionnaires have been set up to appear, you can do this by following these instructions;

  1. Scroll down to the bottom of the dashboard
  2. Click 'Edit' as per the scheme you require to edit
  3. Scroll to the bottom of the page to the Questionnaire section and select 'Edit' to which questionnaire you would like to amend the days after participating on
