Pathways Functionality
What is Pathways?
Pathways allows users to categorise or ‘label’ a referral based on specific characteristics, needs, or assessment criteria. Whether you're running an Exercise Referral Scheme, Mental Health Programme, or any other community health initiative, Pathways enables you to tailor the experience to the individual - and improve outcomes across the board. With Pathways, you can now: define custom categories such as Beginner, Intermediate, or Advanced, restrict access to specific activities based on the referral’s pathway, generate Process Reports filtered by Pathway to monitor trends and performance, easily filter and manage lists to quickly find the right referrals within each scheme. This feature is included at no additional cost for all customers.
To access Pathways, please contact our Support Team and let us know the Pathway titles and which scheme you would like them applied to.
How It Works
Selecting a Pathway
Once your chosen Pathways have been added, simply select ‘manage’ on any referral record and you will find the Referral Pathways Configuration section, where you can simply select the Pathway you want to add a referral to. To remove a referral from a Pathway, follow the same process and select the Pathway they are currently on.
Any changes to a referral’s Pathway will trigger the following prompt. This prompt requires you to enter a reason for the change, both when adding or removing referrals form Pathways, which is recorded in the system along with the date of the entry. This is great for auditing purposes!
Filtering Referrals by Pathway
You can filter your referrals in the List Referrals view to only see those in a specific Pathway. Once Pathways is enabled on your scheme, you can select the Pathway(s) you wish to see and your list will be generated, making it easier to manage different groups, such as Beginners vs Advanced referrals.
Generating Process Reports by Pathway
Pathways can also be used as a filter when generating Process Reports, helping you track performance and outcomes for each group within that scheme. Simply open your Reports page, select your scheme and the following options will be available to you:
Outcome Reports cannot be filtered by Pathway.
Limiting Activities by Pathway
You can also limit which activities are available to referrals based on their Pathway. For example, Beginners may only see low-impact activities, while Advanced referrals have access to more challenging options. To do this, you'll need to Edit your Activities.
(Activities -> Manage Activities -> find the correct scheme -> find the activity -> Edit)
Here’s how that plays out on your platform:
