Update to Cardiac Transfer Form

Many of our customers use the Cardiac Transfer Form within their webforms. To bring the form in line with recent changes in BACPR guidance, we’ve made the following updates:

  • CHD Risk Factors has been renamed to CVD Risk Factors.
  • The following additional risk factors have been added:
    • Type 1 and Type 2 diabetes
    • High cholesterol
    • Anxiety
    • Depression
    • Hypertension
    • Family history of CVD

Why does it matter? 

These updates ensure the Cardiac Transfer Form reflects the latest guidance and captures a wider range of relevant cardiovascular risk factors.

How is works

The updated CVD Risk Factors section will appear automatically on webforms using the Cardiac Transfer Form. Users will be able to record the additional risk factors alongside the existing information when completing the form. The additional field will be non-mandatory.

When processing the referral request in the system, you’ll still need to tick these options when saving the referral if they’ve been selected.
