Refer Patient On: a solution to manage multiple patient needs

Our Refer-On feature enables you to easily move patients between your different schemes. This article provides details of how to set up Refer-On both within your own organisation and externally. 

Referring between schemes within your own organisation

To set up the Refer-on feature, please raise a support ticket and provide the following information about which schemes you want to refer from/to:

From:

Scheme provider, Scheme name

To: 

Scheme provider, Scheme name

Please also state whether you would like to be able to refer in both directions or just from one scheme to another, as these are separate configurations. 

You can use this feature to refer patients between your own schemes, or to/from another organisation's schemes. Simply reach out to our Support Team and they will walk you through the process of setting this up.

When considering this solution, please bear in mind if you are thinking of referring the patient onto another ReferAll scheme, with the intention of deleting the original referral this cannot be actioned. All referrals in all schemes will be deleted for the patient /client.

  1. Complete the steps below to refer a client / patient on to one of the schemes set-up in your ReferAll platform:
  2. Search for the patient and choose the referral record (this could be more than one for a patient/client).
  3. Select the referral.
  4. Select 'Refer Patient On' button.

  5. A pop-up window will display 'Refer this patient on to another service'.
    Select the service to which to refer the patient: (select from scheme options).
    Add a reason for referring on to the selected service (non-mandatory).
    Add a date to 'Date of this change of status'.
  6. Select the 'Refer Patient On' button when you are ready.
  7. Within the same referral record, select the 'Onward Referrals' button.

  8. Scroll to the bottom of the referral record to view the details of the onward referral and click on the Manage Referral button to open the new referral.
     
    The various sub-pages (Inward/Outward/Cross) now have links to the new referral/s. 
    For example, to open the onward referral just click on the Manage Referral button in the Referred onward section of the record.

*Important note*

Scheme Administrators (users) may vary from scheme to scheme, which is why referral ownership defaults to 'Not Owned' in the receiving scheme who will then go forwards and assign referral ownership.

During the configuration process,  if two service organisations or more are partnering, our team will provide you with a checklist which includes - but is not exclusive to -  patient data consent statements and your referring organisations need to match (in your ReferAll platforms) to enable the Refer On solution.

How are completed questionnaires handled?

When referring on, any completed questionnaires are not automatically transferred to the new referral / scheme.
Why is this?
In ReferAll, questionnaires are associated with a referral and scheme. Each time a 'referral' is referred on, a new referral is created in a separate scheme, and only questionnaires related to that scheme are directly available. However, the completed questionnaire/s can still be accessed via the initial Referral ID, details are saved in the Referred On section - and reporting is still available in the original scheme.

Referring to schemes outside of your organisation

To set up Refer-on from your scheme to another organisation's scheme, please raise a support ticket and provide the same information as mentioned at the top of this article. We will then respond by copying in all parties (if they are not already in copy) and requesting confirmation of the configuration required. Next, we will create a form for you to sign via PandaDoc. All parties will receive the form via email and, upon completion, we will set up the configuration. We will send you a final email asking you to confirm you are happy with the new configuration, to which you have 7 days to respond. At this point, the ticket will be closed off and any further confirgurations will be treated as new requests. 

You can then use the Refer-on function by following the above steps 1-8. 

Please note, both parties in the agreement must be ReferAll customers.
